ABOUT THE SURVEY
Hi! You have been asked to participate in this survey in an effort to collect the information of possible plasma donors. This survey is being conducted by Jnana Prabodhini Foundation and is driven by a team of volunteers from India, the US, the UK, and Canada. Results of this data collection drive will be made available to the public on our website so that the donors can be reached out to directly by the people in need.

If you decide to participate, you will be filling out an online survey with some personal information and contact details that will be used to create a plasma donors directory.

The survey will take not more than 5 minutes - 10 minutes to complete.
